General / Left mouse direction always registering

Hi,
Just joined and they were the trickiest verification questions ever! Still not sure what the AM icon depicts (it's not a Prism apparently!).
I've got an issue on my JAMMA cab where AM/MAME is always seeing the left mouse direction, so if I try to define a new key in the AM menu then it straight away sets it to Mouse Left. I've checked in windows and the mouse works normally and isn't always pulling left.
I'm using a JAMMA2PC adapter for the controls and a USB KBM for any Windows inputs, there's nothing on the JAMMA adapter for Mouse left that could be the cause, also I don't have a Mouse Left issue when in Windows. AM is set to Windows Shell on logon so explorer and Windows are not fully running in the background.
Any ideas why this is happening?
